Non-high density lipoprotein cholesterol: a superior cardiovascular risk factor?

Abstract

Dyslipidemia is the single most important risk factor for development of coronary heart disease (CHD) and low density lipoprotein cholesterol (LDL-C) has been considered as the key risk factor for causing atherosclerosis and a primary target for lipid lowering therapy. However, there is a significant residual risk of 70–80% even after intensive LDL-C lowering by statins. Recently, it has been shown that non-HDL-C (which includes all atherogenic lipoproteins) has a distinct advantage over LDL-C in predicting CHD, especially when triglycerides are high. Non-HDL-C is simple to calculate and can be estimated in non-fasting state. Numerous epidemiological studies have demonstrated the superiority of non-HDL-C over LDL-C in predicting CHD and residual risk. Hence, many recent lipid guidelines have suggested that HDL-C must be calculated for prediction of cardiovascular risk and a treatment target, especially when triglycerides are high.
